PARENTS: You must either have the completed registration form returned
by the DEADLINE
to the Friends of Historic Boonville
at 617 East Morgan Street
or you may mail it to:
Missouri River Youth Theatre,
1026 Eleventh Street, Boonville, MO 65233.
If you would prefer to register by phone,

and we can fill out the form for you
and answer any questions you may have.
There will also be a
RULES/MEDICAL RELEASE/CONSENT FORM
to be signed by a parent or legal guardian
before participation in the workshops is allowed,
so if you are not able to come in
with your child early on the first day of workshops and sign this,
please be sure you have called early
to have one mailed to you
or picked one up at the Friends of Historic Boonville!
(You will receive two copies, one to keep for reference
and one to sign, date and return to MRYT.)
